Job summary

Normandy Nursing Center seeks a Social Services Assistant to coordinate discharge planning, assess residents, and support resident rights within the care team. You will collaborate across departments to ensure smooth admissions and high resident satisfaction.

Responsibilities include psychosocial assessments, developing care plans, and documenting actions while promoting a respectful, resident-centered environment. English proficiency is required; bilingual skills are a plus.

Qualifications

• Bachelor's degree in social work is preferred.
• Current state clinical social work licensure is required.
• Two years of experience in Social Services background or a behavioral setting is preferred.

Responsibilities

• Coordinate social service/discharge planning within the hospital system.
• Develop measurable plans to enhance social service delivery.
• Perform psychosocial assessments and reassessments of residents.
• Assist with admissions and maintain proper paperwork and resident records.
• Promote residents' rights and collaborate with families in care planning.
