Job summary

Great Lakes Management Co. in Saint Paul, MN is seeking a Resident Assistant to provide hands-on physical and emotional care under resident service agreements.

Responsibilities include assisting with ADLs, supporting dining and activities, and communicating changes to the case manager while documenting daily care and medication administration as prescribed by physicians.

Qualifications

• Six months of experience in long term care or assisted living preferred.
• Registered Nursing Assistant (CNA) or CNA certification preferred.
• Ability to make responsible choices in residents' best interests.

Responsibilities

• Assist residents with daily living activities and comfort needs as per service plan.
• Support residents in dining and participate in activities and outings.
• Provide physical and emotional support and monitor changes in condition.
• Assist with treatments delegated by the Registered Nurse per service agreement.
• Observe residents and report changes to the case manager.
• Interact with residents to provide reassurance, engagement and redirection.
• Complete required documentation daily and maintain care records.
• Medication administration per MD orders and guidelines.

Resident Assistant Job Objective(s):

The Resident Assistant is responsible for providing hands -on care, both physical and emotional, as outlined in each resident’s service agreement. R esponsible for effectively communicating with residents, families, and notifying the case manager of any changes in condition or concerns. The Resident Assistant assist s in the dining room and participating in activities and outings .

Resident Assistant Job Responsibilitie s :

• Assistresidents with ADL’s (Activities of Daily Living) asindicatedon the individualized service agreement


• Help each resident to reach andmaintainthe highest level of performance and independence.


• Provide residents with both physical and emotional support.


• Assistwith treatments as delegated by the Registered Nurse per the resident service agreement


• The Resident Assistantobservesclientsand reportschanges in physical and emotional condition such as change in attitude, loss of appetite, weight, etc.tothe casemanager.


• Interactwith residents to provide reassurance, engagement, and redirection as appropriate.


• Complete all required documentation daily, service schedule, flow sheet, communication book, etc.


• Medication Administration per MD orders and per guidelines/policy.

Qualifications :

• Six months of experience in long term care or assisted living residence (Preferred)


• Registered as a Nursing Assistant, Home Health Aid or Certified Nursing Assistant (Preferred)


• Ability to make responsible choices and decisions and act in the resident’s best interests.
