Job summary

Fairview Health Services seeks dedicated caregivers to support residents in a senior living setting. You will assist with daily activities, hygiene, meals, and mobility, under RN direction. This role emphasizes compassionate care, safety, and timely reporting of health changes.

Shifts include evenings and weekends in a Lakeville, MN community, offering a supportive team environment and opportunities to help residents maintain independence and dignity.

Qualifications

• Less than 1 year of caregiving or related experience.
• Ability to use computers, handheld devices, phones, and fax machines.
• If under 18 must be MN NA registry and cannot pass meds or operate lifts independently.

Responsibilities

• Assist residents with personal care tasks: bathing, dressing, grooming, and nail care.
• Support feeding, toileting, and mobility including transfers and use of lifts.
• Monitor health concerns per care plan and report to RN; administer meds under nurse direction.
• Provide light housekeeping, meal delivery and service in dining areas.
• Support residents during activities and respond to emergencies and changes in condition.

Job Overview

The Fountains at Hosanna is conveniently located in Lakeville adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. The Fountains and their exceptional staff are known for creating a warm, friendly, fun community.

Week 1: Tues, Wed, Thur, Sat & Sun - 4pm-8pm

Week 2: Mon, Tues, Wed & Thur - 4pm-8pm

This position is responsible for providing personal care and designated health-related services to enhance and/or maintain the residents' physical and emotional wellbeing.

Responsibilities

• Assists residents with personal cares including but not limited to:
• Bathing/showering
• Foot and fingernail care
• Shaving
• Dressing
• Assistance with use of hearing aids and/or glasses
• Hair care
• Feeding
• Toileting
• Escorts
• 1-2 person transfers and use of mechanical lift
• Provides health-related services as directed: Assists with treatments per individualized care plan under the direction of the Registered Nurse. (includes blood glucose monitoring)
• Administers medication or assists with this under the direction of the Registered Nurse (includes insulin injections). Takes and records temperature, blood pressure, pulse and respiration under the direction of the Registered Nurse
• Performs specified home services:
• Performs laundry service per schedule.
• Follows cleaning schedule for the shift worked.
• Delivers meal trays.
• Assists with meal service in dining room.
• Assists with preparing for and/or facilitating resident social and recreational activities as assigned and as requested.
• Observes tenants/residents and reports status to appropriate staff.
• Notifies appropriate staff of changes in physical and/or emotional conditions such as change in attitude or mood, loss of appetite, change in weight.
• Serves as a mandated reporter and reports any concerns regarding potential mistreatment
• Offers pertinent observations and input.
• Responds appropriately to resident and building emergencies.

• If under 18 must be on the MN NA registry and site must be able to manage restrictions that they are not able to pass medications or operate a mechanical lift independently.
