Job summary

Ebenezer in Lakeville, MN is seeking a Resident Assistant/Home Health Aide to provide personal cares and support for seniors in independent living, assisted living, and memory care. Schedule includes 18 hours every two weeks (part time), evenings 4pm–10pm, with a two‑week block pattern and weekend/holiday rotation.

Shift differential pay is available. The role involves daily living assistance, health‑related support under RN supervision, observation and reporting, and following safety protocols

Qualifications

• Less than 1 year of experience.
• Ability to use technology (computers, handheld devices, cell phones, fax machines).
• If under 18, must be on the MN NA registry.

Responsibilities

Job Overview

The Fountains at Hosanna! is conveniently located in Lakeville, adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. We are known for our exceptional staff, who create a warm, friendly, fun community. We are looking for a Resident Assistant/Home Health Aide who would provide personal cares to improve and/or maintain the senior assisted living residents’ physical and emotional well-being for our senior living in Lakeville, MN.

This caregiver schedule includes:

• 18 hours every two weeks ; Part Time
• Evening Short Shift ; 4pm-10pm
• Block Schedule:
  • Week 1: No Shifts
  • Week 2: Friday , Saturday , and Sunday
• Shift differential pay available
• Every other weekend and holiday rotation

Responsibilities

• Personal Care Assistance
• Supports Residents With Daily Living Activities, Including
  • Bathing
  • Foot and fingernail care
  • Shaving
  • Dressing
  • Hair care
  • Feeding
  • Toileting
  • Assistance with hearing aids and/or glasses
  • 1-2 person transfers and use of mechanical lifts
• Health-Related Support
  • Assists with treatments as outlined in individualized care plans under the supervision of a Registered Nurse (e.g., blood glucose monitoring).
  • Administers or assists with medications, including insulin injections, as directed by a Registered Nurse.
  • Takes and records vital signs such as temperature, blood pressure, pulse, and respiration.
• Daily Living Support
  • May assist with household tasks including laundry, cleaning, meal service, and resident activities.
• Resident Observation & Reporting
  • Monitors and reports changes in residents’ physical or emotional condition (e.g., mood, appetite, weight).
  • Communicates observations to appropriate staff and contributes relevant input to care planning.
  • Acts as a mandated reporter, promptly reporting any concerns related to potential mistreatment.
• Emergency Response
  • Responds appropriately to resident needs and building emergencies, following established protocols.
