Ebenezer, operating The Fountains at Hosanna! in Lakeville, MN, is seeking a Resident Assistant/Home Health Aide to provide personal cares and support for seniors in assisted living and memory care.

The role features an 18-hour biweekly schedule (evenings 4pm–10pm), a block schedule with Week 2 Fri–Sun shifts, and shift differential pay. Join a compassionate team dedicated to resident well-being.

Qualifications

• Less than 1 year of experience required.
• Ability to use technology such as computers, handheld devices, cell phones, and fax machines.
• If under 18, must be on the MN NA registry.
• Preferred: 1–3 years of experience.

Responsibilities

• Personal Care Assistance: supports residents with daily living activities (bathing, nail care, shaving, dressing, hair care, feeding, toileting, hearing aid/glasses assistance, 1–2 person transfers).
• Health-Related Support: assists with treatments under RN supervision and with medications as directed; records vital signs.
• Daily Living Support: assists with laundry, cleaning, meals, and resident activities.
• Resident Observation & Reporting: monitors changes, communicates with staff, contributes to care planning, acts as mandated reporter.
• Emergency Response: responds to resident needs and building emergencies per protocols.

Job Overview

The Fountains at Hosanna! is conveniently located in Lakeville, adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. We are known for our exceptional staff, who create a warm, friendly, fun community.

We are looking for a Resident Assistant/Home Health Aide who would provide personal cares to improve and/or maintain the senior assisted living residents’ physical and emotional well-being for our senior living in Lakeville, MN.

This caregiver schedule includes;

• 18 hours every two weeks ; Part Time
• Evening Short Shift ; 4pm-10pm
• Block Schedule:
  • Week 1: No Shifts
  • Week 2: Friday , Saturday , and Sunday
• Shift differential pay available
• Every other weekend and holiday rotation

Responsibilities

• Personal Care Assistance:
  Supports residents with daily living activities, including:

  • Bathing
  • Foot and fingernail care
  • Shaving
  • Dressing
  • Hair care
  • Feeding
  • Toileting
  • Assistance with hearing aids and/or glasses
  • 1-2 person transfers and use of mechanical lifts

• Health-Related Support:

  • Assists with treatments as outlined in individualized care plans under the supervision of a Registered Nurse (e.g., blood glucose monitoring).
  • Administers or assists with medications, including insulin injections, as directed by a Registered Nurse.
  • Takes and records vital signs such as temperature, blood pressure, pulse, and respiration.

• Daily Living Support:

  • May assist with household tasks including laundry, cleaning, meal service, and resident activities.

• Resident Observation & Reporting:

  • Monitors and reports changes in residents’ physical or emotional condition (e.g., mood, appetite, weight).
  • Communicates observations to appropriate staff and contributes relevant input to care planning.
  • Acts as a mandated reporter, promptly reporting any concerns related to potential mistreatment.

• Emergency Response:

  • Responds appropriately to resident needs and building emergencies, following established protocols.
