Job summary

Ebenezer is seeking a compassionate Personal Care Assistant to support residents at our Edina community in York Gardens. You will help with daily living activities and health-related tasks to promote residents’ well-being in a warm, home-like setting.

The role requires attention to safety, collaboration with nursing staff, and a friendly approach to residents and families. Evenings shifts are available across a rotating schedule.

Qualifications

• Tech proficiency with computers, handheld devices, cell phones, and fax machines.
• Ability to follow care plans and document observations.
• Willingness to learn and adapt to resident needs.

Responsibilities

• Assist residents with personal cares: bathing, dressing, grooming.
• Support with mobility and transfers, including use of mechanical lifts.
• Administer medications under RN supervision and record vitals as directed.
• Assist with meals and dining room service.
• Observe residents and report changes in condition to staff.
• Support activities and social programs for residents.

York Gardens - We have 76 Assisted Living, Memory Care and Enhanced Care apartments in a warm, friendly, convenient community.

Week 1: Thur - 2:30pm-10:30pm

Week 2: Sat & Sun - 2:30pm-10:30pm

This position is responsible for providing personal care and designated health-related services to enhance and/or maintain the residents' physical and emotional wellbeing.

Responsibilities

• Assists residents with personal cares including but not limited to:
• Bathing/showering
• Foot and fingernail care
• Shaving
• Dressing
• Assistance with use of hearing aids and/or glasses
• Hair care
• Feeding
• Toileting
• Escorts
• 1-2 person transfers and use of mechanical lift
• Provides health-related services as directed: Assists with treatments per individualized care plan under the direction of the Registered Nurse. (includes blood glucose monitoring)
• Administers medication or assists with this under the direction of the Registered Nurse (includes insulin injections). Takes and records temperature, blood pressure, pulse and respiration under the direction of the Registered Nurse
• Performs specified home services:
• Performs laundry service per schedule.
• Follows cleaning schedule for the shift worked.
• Delivers meal trays.
• Assists with meal service in dining room.
• Assists with preparing for and/or facilitating resident social and recreational activities as assigned and as requested.
• Observes tenants/residents and reports status to appropriate staff.
• Notifies appropriate staff of changes in physical and/or emotional conditions such as change in attitude or mood, loss of appetite, change in weight.
• Serves as a mandated reporter and reports any concerns regarding potential mistreatment
• Offers pertinent observations and input.
• Responds appropriately to resident and building emergencies.

Required Qualifications

• Ability to use technology in the form of computers, handheld devices, cell phones, and fax machines
• If under 18 must be on the MN NA registry and site must be able to manage restrictions that they are not able to pass medications or operate a mechanical lift independently.

Preferred Qualifications

• 1-3 years experience
