Job summary

Neffsville Nursing & Rehab in Lancaster, PA is seeking a full-time Social Worker to advocate for residents' emotional well-being, guide families, and support transitions into and throughout their stay.

You will complete psychosocial assessments, participate in admissions, provide support during crises, document updates, lead groups, and help grow volunteer programs to enhance resident care.

Qualifications

• Bachelor’s degree in Social Work preferred.
• Knowledge of long‑term care social services practices, regulations, and guidelines.
• Strong communication, empathy, and independent problem-solving skills.

Responsibilities

• Complete social assessments and care plans addressing residents’ psychosocial needs with realistic, actionable goals.
• Participate actively in pre-admission and admission processes.
• Provide compassionate support to residents and families during adjustment, crisis, or periods of heightened need.
• Document behavioral and psychosocial updates in accordance with facility and regulatory requirements.
• Lead individual and group support as needed.
• Develop and grow volunteer programs tailored to resident needs.
• Orient new residents and families to the center to ensure a smooth transition.
