St. John Neumann Center for Rehab and Healthcare in Philadelphia, PA is seeking a compassionate Social Worker (Nursing Home) to join our interdisciplinary team.

You will support patients and residents by assessing social needs, planning interventions, and providing counseling and education to families. The ideal candidate holds a BSW or MSW, is licensed or certified as a Social Worker in Pennsylvania, and has at least two years of experience in long-term care.

Qualifications

• Bachelor's or Master's degree in Social Work from an accredited program.
• Licensure or certification as a Social Worker in Pennsylvania.
• Minimum of 2 years of experience in a long-term care setting.

Responsibilities

• Conduct social work assessments and develop comprehensive plans to address the needs of patients and residents
• Identify and address social, emotional, and psychological issues that may impact a patient's or resident's care
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Provide counseling, support, and education to patients, residents, and their families
• Document all interactions, assessments, and care plans in accordance with facility policies and regulations
• Maintain accurate and up-to-date records of patient and resident interactions and care plans
• Participate in continuous quality improvement initiatives to improve patient and resident outcomes
