Key responsibilities include completing social assessments and care plans, participating in pre-admission processes, offering crisis support, documenting updates, and developing volunteer programs.

Qualifications

• Bachelor’s degree in Social Work or related field preferred.
• Knowledge of long‑term care social services practices, regulations, and guidelines.
• Strong communication, empathy, and independent problem‑solving skills.

Responsibilities

• Complete social assessments and care plans addressing residents’ psychosocial needs with realistic, actionable goals.
• Participate actively in pre-admission and admission processes.
• Provide compassionate support to residents and families during adjustment, crisis, or periods of heightened need.
• Document behavioral and psychosocial updates in accordance with facility and regulatory requirements.
• Lead individual and group support as needed.
• Develop and grow volunteer programs tailored to resident needs.
• Orient new residents and families to the center to ensure a smooth transition.
