Job summary

Carmel Hills Wellness & Rehabilitation is seeking a compassionate and driven Social Services Designee to support the emotional, social, and psychological well-being of our residents and their families in a skilled nursing environment.

You will perform psychosocial assessments, contribute to individualized care plans, advocate for resident rights, and coordinate transitions into long-term and memory care with the interdisciplinary team.

Qualifications

• Certified, licensed, or registered in the state of practice (required)
• Bachelor’s degree in Social Work or a related Human Services field
• Minimum of 1 year of social work experience in a healthcare or geriatric setting
• Skilled Nursing Facility (SNF) experience preferred
• Nursing experience in a SNF is a plus

Responsibilities

• Conduct psychosocial assessments and contribute to individualized care plans
• Support residents and families through transitions into long-term care and memory care
• Complete MDS-related documentation accurately and in compliance with regulations
• Advocate for resident rights, dignity, and self-determination
• Coordinate discharge planning and connect residents with community resources
• Collaborate with nursing, administration, and the interdisciplinary team
• Assist with survey readiness and regulatory compliance efforts

Education

Bachelor’s degree in Social Work or related Human Services field
