Job summary

American Medical Administrators MGMT Company is seeking a Social Services Director to oversee resident and family social services in our long-term care facility. You will conduct psychosocial assessments, assist with admissions, and coordinate services with medical staff, clinicians, and external partners.

Requirements include a Bachelor's degree in social work or related field and at least one year in a healthcare setting as a supervised social worker, with strong communication and teamwork

Qualifications

• Bachelor's degree in social work or related major.
• At least 1 year in a healthcare setting as a supervised social worker.
• Ability to communicate effectively and work within a team.
• Knowledge of available community resources.

Responsibilities

• Provide social services to residents and families under the direction of the Social Services Director.
• Complete Initial Psychosocial Assessment and follow up visits; may do some MDS assessments under supervision.
• Arrange purchases for residents and ensure adequate clothing.
• Schedule visits to dental, eye, podiatrist, and audiologist.
• Provide support in times of loss to residents and families.
• Assist residents in gathering documents for Medicaid, Medicare, SSI, etc.
• Other duties as directed by the Social Services Director.
