Job summary

Responsibilities
- Conduct psychosocial assessments and update care plans.
- Provide counseling and supportive services to residents and families.
- Assist with admission, discharge, and transfer processes, ensuring communication with physicians and families.
- Facilitate discharge planning, including coordination with community resources and services.
- Advocate for resident rights and educate staff on psychosocial issues.
- Document social services interventions and progress in resident records.
- Participate in interdisciplinary care conferences and survey preparation.
