Job summary

Blue Central Group is seeking a dedicated Social Services Designee or Licensed Social Worker (LSW) to support residents in a skilled nursing and long-term care setting. You will advocate for residents’ rights and psychosocial needs, coordinating care with nursing and therapy teams to maintain quality of life.

You will complete assessments, develop care plans, and assist with discharge planning while ensuring confidentiality and HIPAA compliance.

Qualifications

• Must meet qualifications for Social Services Designee or hold an active Ohio LSW license.
• Strong communication, organizational, and interpersonal skills.
• Ability to work with residents, families and interdisciplinary teams.

Responsibilities

• Assess psychosocial, emotional, and social needs of residents and address them.
• Advocate for residents' rights, dignity, preferences, and independence.
• Provide emotional support to residents and families during transitions in care.
• Complete social work assessments, documentation, and care plans accurately and timely.
• Participate in interdisciplinary care planning meetings and coordinate discharge planning.
