Job summary

SLP Operations is seeking a licensed Social Worker to lead the social services efforts in a long-term care setting. The role focuses on resident advocacy, coordination with the Administrator, and compliance with federal, state, and local standards.

Responsibilities include directing social services, planning discharges, and ensuring thorough documentation, while maintaining resident confidentiality and dignity in care. Evening or irregular hours may be required.

Qualifications

• Must be a licensed Social Worker in good standing with the state.
• Able to interact positively with residents, families, and regulatory agencies.
• Ability to work long or irregular hours under pressure.

Responsibilities

• Develop, implement, and direct all social services functions.
• Coordinate social services activities with other departments and implement changes as required.
• Ensure forms, reports, and progress notes are properly completed and informative.
• Participate in discharge planning and community planning to meet residents' needs.
• Coordinate programs with residents, families, staff and community agencies.
• Provide information to families about support agencies and financial assistance.
• Maintain resident confidentiality and uphold residents' rights.
• Monitor emotional and social needs of residents and staff awareness.
• Perform other duties as assigned.
