Job summary

Atlas Healthcare, LLC seeks an experienced Social Services Director to plan, organize, and direct the Social Services Department within a skilled nursing and rehabilitation facility. This role advocates for residents’ psychosocial well-being, coordinates admissions and discharges, and collaborates with an interdisciplinary team to improve care outcomes.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field preferred.
• Minimum 2 years of social services experience in a skilled nursing facility, rehabilitation center, hospital, or long-term care setting preferred.
• Current Social Work license as required by state regulations.
• Nursing Home Social Services certification or equivalent, if required by state law.

Responsibilities

• Develop departmental goals, policies, and procedures.
• Manage social services staff, including scheduling, training, and performance evaluations.
• Coordinate admissions and assist residents and families with transition planning.
• Facilitate safe and effective discharge planning, coordinating with nursing, therapy, physicians, and community resources.
• Advocate for resident rights, dignity, privacy, and self-determination.
• Attend resident care conferences and interdisciplinary team meetings.
• Maintain accurate documentation in the medical record and participate in QAPI initiatives.
