Job summary

Springfield Skilled Health Care seeks a Social Services Director to lead the social services team in the Greater Springfield Area, IL. You will support residents' emotional, psychological, and social needs, advocating for dignity and quality of life.

The role emphasizes building relationships with residents, families, and staff, coordinating care, and ensuring resources are connected to those in need.

Qualifications

• Social services experience in a skilled nursing or long-term care setting preferred.
• Knowledge of MDS process and psychosocial assessments preferred.
• Strong communication, organization, and interpersonal skills.
• Compassionate, dependable, and team-oriented attitude.

Responsibilities

• Oversee and manage the daily operations of the Social Services Department.
• Identify and address residents' psychosocial, emotional, and social needs.
• Complete social services assessments and develop individualized care plans.
• Participate in MDS assessments and documentation.
• Coordinate and facilitate resident care conferences with families and interdisciplinary team members.
• Build relationships with residents, families, and responsible parties.
• Coordinate resident discharges, including home health services and transfers.
• Advocate for residents while ensuring rights, dignity, and well-being are respected.
• Connect residents and families with appropriate resources and support services.
