Complete Care in Madison, WI is seeking a Social Services Director / Social Worker to advocate for residents and drive psychosocial care. You will assess needs, document thoroughly, and work within the interdisciplinary team to create patient-centered plans and ensure rights and dignity are upheld.

The role requires a BSW or MSW with an active WI license, plus 1 year of supervised health-care social work experience.

Formación

• Education: BSW or MSW required; licensure in Wisconsin.
• Active social work license (CSW/APSW/LCSW or equivalent) in WI.
• Minimum 1 year supervised social work in healthcare; LTC/SNF preferred.
• Strong communication, documentation, and team collaboration skills.

Responsabilidades

• Assess residents' psychosocial, mental, and emotional needs through regular visits.
• Complete Social History and psychosocial assessments on admission and regularly thereafter.
• Integrate findings into the interdisciplinary care planning process (MDS).
• Advocate for residents' rights, dignity, and self-determination.
• Prevent and address abuse, neglect, and exploitation as required by law.
• Coordinate discharge planning and link to post-discharge resources.
• Provide emotional support and counseling to residents and families.
• Per Administrator's direction, perform other duties as assigned.
