Job summary

Care Plus Management is seeking a Social Services Director to lead social services for residents, coordinating with nursing, activities, dietary, therapy, and administration to ensure psychosocial needs are identified and addressed. The role emphasizes resident rights, advocacy, and regulatory compliance.

The director will oversee care planning, admissions, discharge planning, family relations, and quality improvement, ensuring documentation is thorough and HIPAA-compliant.

Qualifications

• Current Licensed Clinical Social Worker (LCSW) license in the State of Illinois, in good standing.
• Bachelor’s degree in Social Work from an accredited program.
• Previous experience in social services within long-term care, healthcare, or geriatric settings preferred.
• Knowledge of long-term care regulations, resident rights, discharge planning, and social services practices.
• Strong assessment, counseling, communication, and problem-solving skills.

Responsibilities

• Conduct psychosocial assessments for residents on admission and as required.
• Identify residents’ psychosocial, emotional, behavioral, social, and adjustment needs.
• Develop and implement individualized social service interventions based on resident needs and goals.
• Provide counseling and support to residents experiencing adjustment difficulties, grief, or behavioral concerns.
• Participate in interdisciplinary care plan meetings and communicate recommendations.
• Promote and protect residents’ rights and advocate for residents.
• Coordinate with hospitals and community resources for discharge planning and transitions.
