Job summary

Atrium Centers Blossom Nursing & Rehabilitation Center is seeking an Admissions & Social Services Director to lead the admissions process and support residents’ psychosocial needs. You will partner with hospitals, referrals, and the interdisciplinary team to ensure smooth transitions and quality outcomes.

In this role you’ll coordinate documentation, payer verification, and regulatory compliance while developing social programs and maintaining Residents’ Rights.

Qualifications

• Must be a Licensed Social Worker (LSW) as required by state regulations.
• Bachelor’s Degree in Social Work, Human Services, or related field preferred.
• 1–3 years of healthcare, long-term care, admissions, or social services experience preferred.
• Knowledge of long-term care regulations, reimbursement systems, and admissions processes.
• Experience with Medicare, Medicaid, and managed care guidelines.
• Strong communication, counseling, and interpersonal skills.
• Excellent organizational, problem-solving, and decision-making skills.
• Ability to build relationships with residents, families, referral sources, and community partners.
• Proficient with Microsoft Office and electronic documentation systems.
• Willingness to work flexible hours, including evenings or weekends as needed.
• Ability to travel locally; reliable transportation required.
• Must meet all pre-employment requirements, including background screening and health clearance.
• Able to perform essential job functions with or without reasonable accommodation.

Responsibilities

• Coordinate and manage the admissions process from initial inquiry through transition into the center.
• Respond to hospital, community, and family inquiries and conduct tours and pre-admission assessments.
• Coordinate admissions with nursing, business office, and interdisciplinary departments.
• Ensure completion of admission documentation, payer verification, and regulatory requirements.
• Monitor referral activity, admissions trends, and census data and prepare reports.
• Communicate projected admissions, discharges, and bed changes to departments.
• Develop, implement, and evaluate social service programs addressing residents’ psychosocial needs.
• Complete social histories, assessments, and care plan contributions per regulations.
• Provide counseling, support, and education to residents and families.
• Coordinate services including dental, vision, behavioral health, and community resources.
• Participate in care planning, behavior management, and interdisciplinary meetings.
• Assist with discharge planning from admission through transition for continuity of care.
• Educate residents and families on advance directives, financial resources, and services.
• Promote Residents’ Rights and support grievance resolution processes.
• Support Resident and Family Council activities.
• Build relationships with hospitals, case managers, providers, and referral sources.
• Engage in community outreach, presentations, and networking to support census development.
• Collaborate with center leadership on marketing strategies and admissions planning.
• Maintain PHI confidentiality per HIPAA.
• Participate in quality improvement initiatives and admissions committees.
