Atrium Centers in Ohio is seeking an Admissions & Social Services Director to oversee the admissions cycle and support residents’ psychosocial, emotional, cultural, and adjustment needs. You’ll collaborate with hospitals, families, and the interdisciplinary team to ensure smooth transitions and quality outcomes.

The role requires an LSW or equivalent, a bachelor’s in social work or related field, 1–3 years in healthcare, and strong communication, organization, and relationship-building skills.

Qualificações

• Requires Licensed Social Worker (LSW) or equivalent.

Responsabilidades

• Oversee admissions process from inquiry to transition into center.
• Coordinate with nursing, business office, and interdisciplinary teams for smooth transitions.
• Ensure completion of admission docs, payer verification, and regulatory requirements.
• Monitor referrals, census, and admissions trends; prepare related reports.
• Support social histories, care plan contributions, and PHI-compliant documentation.
• Provide counseling and education to residents and families.
• Collaborate with hospitals and community partners for outreach and census development.
• Contribute to marketing strategies and admissions planning.
