Job summary

Atrium Centers in Ohio seeks an Admissions & Social Services Director to welcome new residents and support their psychosocial, emotional, cultural, and adjustment needs.

You’ll collaborate with hospitals, referral sources, and the interdisciplinary team to ensure smooth transitions, strong census performance, and high-quality care.

Qualifications

• Licensed Social Worker (LSW or equivalent) as required by state regulations.
• Bachelor’s Degree in Social Work, Human Services, or a related field preferred.
• 1–3 years of healthcare/social services experience preferred.

Responsibilities

• Coordinate and manage the admissions process from inquiry to transition into the center.
• Respond to hospital, community, and family inquiries and conduct tours and assessments.
• Coordinate admissions with nursing, business, and interdisciplinary teams for smooth transitions.
• Ensure completion of admission documentation, payer verification, and regulatory requirements.
• Monitor referral activity, admissions trends, and census data; prepare related reports.
• Communicate projected admissions, discharges, and bed changes to relevant departments.
• Develop, implement, and evaluate social service programs addressing residents’ needs.
• Complete social histories, assessments, and care plan contributions per regulations.
• Provide counseling, support, and education to residents and families.
