Job summary

PACS is seeking an Activity Assistant to support the Activity Director in delivering a creative and interactive activities program for residents, aligned with federal, state, local, and corporate guidelines. Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read technical procedures and follow policy manuals.
• Ability to present information clearly to managers and employees.

Responsibilities

• Participate in planning and conducting of individual, small and large group activities.
• Assist in communication between employees, residents, families, and the public to ensure needs are met.
• Assist in developing monthly activity calendar and maintaining attendance records.
• Assist in discharge planning and development of activity care plans and resident assessments.
• Arrange transportation for residents for outings, appointments, or discharge.
• Encourage participation in self-initiated activities and provide needed materials.
