Job summary

DEERCREEK is seeking an Activity Assistant to support the Activity Director in delivering a creative, engaging program for residents. You will help plan and run activities that meet residents’ interests and abilities, following all regulatory and company guidelines.

You will work closely with staff, residents, families, and community partners to coordinate calendars, track attendance, and assist with assessments and discharge planning as needed. This role emphasizes communication and compassion.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.

Responsibilities

• Plan and conduct individual and group activities.
• Facilitate good communication among staff, residents, families and the public.
• Develop monthly activity calendar and track attendance.
• Assist in developing and implementing activity care plans and resident assessments.
• Coordinate transportation for residents to outings and appointments.
• Assist with discharge planning and community planning related to resident interests.
• Maintain a clean, orderly Activity Department.
• Encourage resident participation in self-initiated activities.
