Job summary

ChristianaCare is hiring a full-time Registered Nurse, Renal Transplant Coordinator for the Kidney Transplant Department in Newark, DE. This role leads coordination of education, evaluation, waitlist management, and after transplant care for adult patients, working with a multidisciplinary team to ensure excellent transplant outcomes.

The RN acts as a primary contact for candidates, living donors, physicians, and hospital staff throughout the transplant process, including donor organ

Qualifications

• B.S./BSN in Nursing required, MSN preferred.
• A minimum of three (3) years of registered nursing experience is required, preferably in Transplantation, Critical Care, Case Management, Nephrology, Dialysis, or related specialty.
• Basic Life Support (BLS) required

Responsibilities

• Coordinate diagnostic testing and procedures, monitor patient outcomes, and ensure compliant communication among patients, families, providers, and external organizations.
• Provide 24/7 on-call transplant coordination support and assist with discharge planning and resource referrals.
• Serve as a clinical liaison throughout the transplant journey, collaborating with a multidisciplinary team to manage evaluations, readiness, and post‑transplant follow-up.
