Job summary

Wheeler is seeking a Peer Specialist to join the Access Mental Health team, linking children and youth with behavioral health supports and coordinating referrals. You will make outreach calls to families, monitor follow-up, and support engagement with appointments.

Position is primarily remote with an office in Plainville, CT. Full-time; a High School Diploma and CT certification are required, Spanish preferred.

Qualifications

• High School Diploma plus some personal or family experience navigating behavioral health services.
• Direct experience supporting youth and families preferred.
• Spanish speaking preferred.
• Certification in Connecticut required when available.

Responsibilities

• Provides assistance and coordination of care to parents accessing behavioral health care for their children and youth.
• Collaborates with other providers involved in the youth’s care to obtain needed services.
• Attends meetings to facilitate coordination of complex services and resources.
• Documents and tracks coordination efforts via computer-based systems.
• Maintains relationships with community-based advocacy and collaborative groups.
