Job summary

Wheeler Clinic seeks a Peer Specialist to link children and youth with behavioral health supports. You will make follow-up calls to families, coordinate care, and connect them with traditional and non-traditional referrals to ensure engagement and appointment adherence.

The role emphasizes collaboration with health providers, attendance at community meetings, and ongoing education of families about available resources. Remote work options available.

Qualifications

• High School Diploma required; CT certification required when available.
• Direct experience supporting youth and families preferred.
• Spanish speaking is preferred.

Responsibilities

• Provide telephonic or in-person coordination to families seeking behavioral health services.
• Collaborate with healthcare providers to facilitate access to care for children and youth.
• Attend coordination meetings to align services and resources.
• Educate families on resources and processes for accessing behavioral health services.
• Document coordination efforts and communicate with related staff.
• Build relationships with community-based advocacy and collaborative groups.
• Maintain professionalism and customer service in all interactions.
