Job summary

Infinity Hospice Care LLC is seeking an experienced Clinical Care Manager (RN) to serve as the clinical contact for a home care caseload. The role blends nursing judgment with hands-on care coordination, triaging medical concerns, and resolving caregiver performance and scheduling issues within the home care setting.

The ideal candidate is a licensed RN with 2–3 years of clinical nursing experience, preferably in home health or hospice.

Qualifications

• Active RN license in the state of practice.
• 2–3 years of clinical nursing experience; home health or hospice preferred.
• Experience with EVV or care management software is a plus.

Responsibilities

• Triage symptoms and determine urgency and response.
• Evaluate changes in condition and vital signs; respond appropriately.
• Provide guidance after incidents and document appropriately.
• Assess medication administration decisions with prescriber parameters.
• Monitor antibiotic questions and escalate as needed.
• Oversee case management for medication coordination.
• Coordinate with providers, dialysis centers, and external teams.
• Communicate with tribal/insurance case managers regarding care plans.
