Job summary

Infinity Hospice Care LLC administers nursing care for clients in their homes, coordinating with the interdisciplinary team, clients and their families, and referring agencies. This role emphasizes assessments, care planning, and ongoing coordination to support patient recovery and comfort.

The RN will ensure infection control, communicate with care teams, and participate in case conferences while meeting licensure requirements and travel locally to patient homes.

Qualifications

• One (1) year experience as an RN in a clinical care setting.
• Preferred: Home health experience.
• Current State RN license in good standing and ability to renew.
• Current CPR certification.
• Reliable transportation and driver’s license terms.
• Ability to travel locally and work a flexible schedule.

Responsibilities

• Promote the Company's mission, vision, and values.
• Complete initial and ongoing assessments to identify needs and complete OASIS as needed.
• Regularly reevaluate nursing needs and outcomes; assist recovery or maintenance of life functions.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care; plan for discharge.
• Provide effective communication to clients, families, and team members; participate in Case Conferences.
• Apply infection control and universal precautions in coordinating care (OSHA).
• Coordinate services and inform physician and staff of changes in condition.
• Monitor cases for third-party payer requirements.
• Complete documentation timely to ensure policy compliance.
• Supervise and provide clinical direction to other nursing personnel.
• Perform on-call responsibilities as assigned.
• Participate in in-service programs and maintain competency.
• Meet mandatory continuing education requirements.
