Job summary
Infinity Hospice Care is seeking a Registered Nurse Case Manager to coordinate admissions and provide skilled nursing care for patients in their homes. You will work with the Interdisciplinary Team, patients, families, and referring agencies to ensure comprehensive care and smooth transitions.
Requirements include 2 years RN clinical experience, 1 year hospice, and an active RN license. Travel to patient homes is required as part of a flexible schedule and on-call rotation, with a supportive
Qualifications
- Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Current State RN license and valid driver’s license.
- Knowledge of state and federal regulations; strong interpersonal skills.
Responsibilities
- Assess hospice patients using Nursing Assessment and LCD criteria.
- Develop and document the plan of care.
- Provide nursing care with infection control measures.
- Coordinate with Interdisciplinary Team to meet patient/family needs.
- Communicate with physicians involved in patient’s care.
- Submit required paperwork promptly.
- Support QAPI and hospice initiatives.
- Maintain productivity standards and on-call responsibilities.
