Job summary

Lifecare-Home-Health-Family-1 is seeking an experienced Clinical Care Manager (RN) in Florida to serve as the clinical point of contact for home care clients and families. Qualifications

• Active RN license in Florida.
• 2–3 years clinical nursing experience; home health/hospice preferred.
• Experience with EVV or care management software.
• Strong triage and clinical decision‑making skills.
• Ability to de‑escalate and communicate with families and staff.

Responsibilities

• Serve as the first contact for clinical calls and triage symptoms.
• Evaluate changes in condition and determine urgency and response.
• Provide guidance on medication administration within scope and parameters.
• Monitor antibiotic questions and escalate to physician as needed.
• Oversee caregiver performance and scheduling in the home setting.
• Coordinate with providers and external agencies on care plans.
• Maintain HIPAA compliance and accurate documentation.
