Job summary

BrightSpring Health Services is seeking a part-time, remote Care Facilitator - Medical Assistant to support Starting Point Nursing Services. The role includes patient engagement, care coordination, and documentation for a 24/7 triage environment.

Responsibilities include coordinating appointments, communicating with patients and providers, and maintaining records in EMR systems. Some college/associate degree in clinical sciences is preferred; MA certification is a plus.

Qualifications

• Two years of experience collecting referral information in healthcare markets
• Medical Assistant certification preferred but not required
• Experience with EMR software and MS Office Suite

Responsibilities

• Engage with patients and families, triage emergent calls to the clinical team
• Coordinate appointments and referrals for timely follow-up
• Facilitate collaboration among healthcare providers and care teams
• Monitor patient progress and report changes to clinical leadership
• Maintain accurate documentation and ensure HIPAA compliance
• Participate in quality improvement initiatives and process enhancements
