Oak Street Health is seeking a Clinical Call Center Triage Nurse to address patients’ medical, social, and psychological needs by telephone, extending care beyond clinic hours and coordinating appointments and transportation.

Qualifications

• Active, non-probationary RN license in OSH practice states.
• Ability to manage time and work autonomously with reliability.
• Proactive maintenance of licenses and CE requirements.

Responsibilities

• Provide professional service to OSH patients over the phone.
• Follow prescription refill policy.
• Document calls clearly and completely.
• Update and close patient requests before ending calls.
• Schedule appointments for off-cadence or requested visits.
• Arrange transportation for clinic visits.
• Monitor CareReq queue and ensure completeness.
• Meet departmental metrics and goals.
• Other duties as assigned.
