Monogram Health is seeking a Travel - Advanced Practice Provider to ensure patients receive comprehensive care in their homes. The role requires an active RN and Nurse Practitioner certification and focuses on chronic conditions management, including CKD.

The successful candidate will travel extensively to provide personalized care, conduct assessments, document patient records, and collaborate with healthcare teams. Benefits include comprehensive medical coverage, retirement support, and flexible leave policies.

Formación

• Must be proficient with medical instruments and equipment required by the work.
• Access to transportation, a valid driver's license, and car insurance.
• Knowledge of federal and state regulations including HIPAA and OSHA.

Responsabilidades

• Conduct in-home assessments, including annual wellness exams.
• Counsel and educate patients about health benefits.
• Coordinate care with primary care providers and specialists.

Conocimientos

Active and unrestricted Registered Nurse and Nurse Practitioner

Ability to travel up to 75% of the time

Bilingual in Spanish

2 or more years of direct patient care

Educación

Board certified for appropriate licensure (NP: ANCC/AANP)
