Job summary

Monogram Health seeks Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to join our integrated team delivering in-home and virtual care for patients with CKD/ESRD. You will work remotely within your employed state, coordinate with physicians, and drive evidence-based care to reduce hospitalizations and improve quality of life.

Key duties include wellness visits, medication management, documentation, and collaboration with interprofessional care teams.

Qualifications

• Active RN/NP/PA license with board certification as applicable
• BLS certification required; onboarding supports certification
• DEA certificate active and unrestricted

Responsibilities

• Conducts assessments including wellness exams in home or virtual settings
• Counsels and educates patients and families about benefits and programs
• Documents medical history, exam findings, diagnoses, plan and medications
• Coordinates care with primary care providers and specialists
• Delivers timely documentation and maintains quality of care
