Monogram Health seeks a Travel - Advanced Practice Provider to deliver comprehensive care to CKD/ESRD patients in home and virtual settings across markets. This role travels up to 75% of the time while remaining remote in-state employment; flexibility to work across multiple markets is required for optimal patient access.

The NP will assess health, educate patients, coordinate with physicians and care teams, prescribe medications, and ensure documentation aligns with HIPAA and quality standards.

Qualifications

• Active and unrestricted RN and NP licensure required.
• Board certification for NP (ANCC or AANP).
• Current DEA certificate and ability to travel up to 75%.
• Ability to work autonomously and communicate effectively.

Responsibilities

• Travel throughout territory to conduct home visits and wellness exams.
• Counsel and educate patients and families about benefits and programs.
• Document medical history, diagnoses, exams, medications and plans.
• Coordinate care with primary care providers and specialists.
• Prescribe medications, order tests, and collaborate with Monogram physicians.
• Maintain timely documentation and uphold quality of care standards.
• Participate in meetings and provide care in home or virtual settings.
