Job summary

Monogram Health is seeking an Advanced Practice Provider (Nurse Practitioner) to deliver comprehensive, in-home care for patients with chronic kidney disease to ESRD within designated territory. You will work with field-based nurses, community health workers, and physicians to assess health, order tests, prescribe medications, and coordinate care across multiple markets.

Qualifications

Active and unrestricted Registered Nurse and Nurse Practitioner.
Board certified for appropriate licensure (NP: ANCC/AANP).
Current and unrestricted DEA certificate.
Ability to travel up to 100% of the time.
Work autonomously without direct supervision.
Access to transportation and valid driver’s license.
Proficient with medical instruments and equipment.
Knowledge of computer-based data management programs and medical records systems.
Effective communication with diverse stakeholders.
Knowledge of HIPAA and OSHA regulations.
Managed Care/IPA/Health Plan experience.
Experience conducting wellness visits or focused assessments virtually or in-home.

Responsibilities

Travel throughout assigned territory for in-home assessments.
Educate patients and families about benefits and programs.
Document medical history, exams, diagnoses, and plans.
Coordinate care with primary care providers and specialists.
Complete documentation in a timely manner.
Maintain quality of care standards per practice guidelines.
Deliver care remotely within state of employment and attend periodic trainings.
Prescribe medications and order tests in collaboration with Monogram physicians.
Improve patient outcomes and close gaps in care.
Participate in integrated care team meetings.
