Job summary

Advocate Health is seeking a part‑time Remote Transitional Care RN to assess high‑risk patients after discharge and during the 30‑day period. You will develop and adjust patient‑centered care plans, coordinate with care teams, and educate patients on disease management and meds from home.

The role requires a BSN and an active RN license with at least five years in acute or transitional care, plus experience with complex patient populations. Must reside in NC within one hour of Mint Hill.

Qualifications

• RN with BSN and an active, unrestricted license.
• Minimum five years in acute, discharge planning, post‑acute or transitional care.
• Experience with complex populations (heart failure, COPD, sepsis, stroke) preferred.

Responsibilities

• Conduct comprehensive clinical assessments for high‑risk patients at discharge and during 30‑day period.
• Create and update patient‑centered care plans addressing clinical needs and meds.
• Coordinate care among inpatient teams, PCPs, specialists, home health, and community resources.
• Educate patients and families on disease management, meds, symptoms and escalation steps.
• Manage caseloads and ensure follow‑up visits, home visits, and treatment adherence.
• Lead interdisciplinary rounds, collaborating with physicians, pharmacists, social workers, and nurses.
