Job summary

Advocate Aurora Health, Inc. is seeking a part-time RN for Transitional Care Management in a fully remote home-based role in Wisconsin. Duties include discharge assessments, care planning, and coordination with inpatient and community teams. Requires BSN, active RN license, and 5 years of clinical nursing experience.

Schedule is 20 hours per week with every Saturday and Sunday coverage and potential holidays. Salary ranges from $38.20 to $57.30 per hour with full benefits eligibility.

Qualifications

• BSN required.
• Active RN license.
• Minimum five years clinical nursing experience in acute or transitional care.
• Experience with complex patient populations including heart failure, COPD, GI bleed, sepsis, or stroke.

Responsibilities

• Perform comprehensive clinical assessments of high-risk patients at discharge and during 30-day transition.
• Develop and update individualized, patient-centered care plans.
• Coordinate care among inpatient and outpatient providers and community resources.
• Educate patients and families on disease management, medications, and escalation plans.
• Manage a caseload with follow-up appointments, home visits, and adherence support.
• Participate in interdisciplinary rounds and quality improvement initiatives.
