Job summary

Advocate Health is seeking a Part-time Transitional Care RN to perform remote clinical assessments and care coordination for high‑risk patients in the 30‑day post‑discharge period. The role requires a BSN and an active RN license, with a strong emphasis on education, communication, and care planning.

The schedule is primarily weekends (Saturday and Sunday 08:00–18:30) with potential holidays, remote work from home, and occasional on‑site meetings as needed.

Qualifications

• BSN required and RN license active and unrestricted.
• Minimum five years in clinical nursing related to discharge or transitional care.
• Three years’ experience with complex patient populations including heart failure and sepsis.

Responsibilities

• Conduct comprehensive clinical assessments of high‑risk patients at discharge and during 30‑day transition.
• Develop and update individualized care plans addressing clinical needs and medications.
• Coordinate care and facilitate communication among inpatient and outpatient teams.
• Educate patients and families on disease management and escalation protocols.
• Manage caseloads with follow‑up visits, home visits, and adherence checks.
• Lead interdisciplinary rounds and participate in quality improvement initiatives.
