Job summary

Advocate Health is hiring a part‑time RN for transitional care management. This fully remote role requires a BSN, an active RN license, and 5+ years in acute/post‑acute/transitional care. The position involves coordinating care across inpatient and community resources, educating patients, and ensuring follow‑ups.

Work is primarily remote with standard weekday hours and occasional evenings/weekends for caseload coverage. 20 hours per week, with a pay range reflecting part‑time compensation.

Qualifications

• BSN required.
• Active, unrestricted RN license.
• 5+ years nursing in acute/post‑acute or transitional care.
• 3+ years education for complex populations (heart failure, COPD, sepsis, stroke).

Responsibilities

• Conduct comprehensive clinical assessments during discharge and 30‑day transition.
• Create and update patient‑centered care plans (clinical needs, meds, follow‑up).
• Coordinate care among inpatient, primary, specialists, home health, and community resources.
• Educate patients and families on disease management and escalation protocols.
• Manage a caseload and ensure follow‑ups, home visits, and treatment adherence.
• Lead interdisciplinary rounds and case conferences with the care team.
• Monitor progress via outreach, telehealth, and calls; address issues promptly.
• Ensure CMS/TCTM and Joint Commission documentation compliance.
• Participate in quality initiatives to improve workflows and outcomes.
• Perform additional duties as assigned.
