Job summary

Carewell seeks a Registered Nurse (RN) Care Advocate for a remote, 1099 contract role in the United States. You will assess members by phone/video, build care plans, and coordinate across providers to close care gaps.

This flexible, self-directed position lets you set hours and work part-time to full-time from a HIPAA‑compliant home workspace. You will leverage your clinical judgment and effective communication to educate and advocate for patients, ensuring safe transitions and timely follow-up

Qualifications

• Active, unrestricted RN license in at least one U.S. state.
• 3+ years of RN experience in care management, transitions of care, home health, or primary care.

Responsibilities

• Conduct phone and video assessments of members' health status, medications, and social needs.
• Build member-centered care plans with goals and next steps.
• Coordinate with providers, pharmacies, and home health agencies to close care gaps.
