Job summary

Humana Inc. is seeking a Transition Coordinator (Care Coach 2) to evaluate member needs and guide families toward appropriate resources to achieve optimal wellness.

The role requires coordinating transitions in Medicaid programs and care settings, including planning, follow-up, and collaboration with care teams. You will work remotely with Indiana-based members, require a valid driver's license, and a private workspace.

Qualifications

• Bachelor's degree in health and human services field.
• 2+ years of relevant experience.
• Proficiency with Microsoft Word, Excel and Outlook.
• Detail oriented with ability to meet tight deadlines.
• Strong verbal and written communication skills.
• Must be patient-facing and able to work in Humana TB screening program.

Responsibilities

• Support and oversee initial and ongoing member transitions in Medicaid programs and care settings.
• Assist with transition planning, preparation and follow-up care.
• Coordinate with Member Advocate Coordinator for continuity of care.
• Transfer and review prior authorization decisions and clinical information.
• Assist with transitions from custodial to home/community-based settings.
• Conduct telephonic and in-person meetings within the assigned region.
• Develop member-specific care plans and coordinate services.
• Guide members toward resources and payer/provider interfaces as needed.
