Qualifications

• Spanish-speaking ability preferred.
• Bachelor's or Master's in social work, psychology or related field; experience acceptable in lieu of degree.
• Strong interpersonal skills to interact with diverse populations and ethical professional conduct.

Responsibilities

• Understands and applies program-specific case management requirements.
• Approves funds for services and develops annual cost budgets as applicable.
• Stays current with Medicare/Medicaid guidelines and related regulations (ADA, waivers).
• Leads development and implementation of the consumer’s care plan and services scope.
• Completes home visits or appropriate contacts and monitors services as required.
