Centene Corporation is hiring a remote Care Manager, Transition of Care, to support SSI and BC+ Medicaid members who are currently hospitalized. The role emphasizes behavioral health coordination with licensed professionals in Wisconsin, requiring strong case management and knowledge of community resources.

The position involves evaluating post-discharge needs, coordinating with PCPs and specialists, and developing care plans to ensure seamless transitions and ongoing wellness.

Formación

• Requires a Master’s degree in Behavioral Health or Social Work or Nursing degree with 2–4 years of related experience.
• Licenses: LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN.
• Care management experience for post-discharge transitions preferred.

Responsabilidades

• Evaluate the needs of the member by completing post-discharge assessments.
• Evaluate medication and perform reconciliation between pre-admit and post-discharge medications.
• Develop a care/service plan and collaborate with discharge planners, providers, specialists, and interdisciplinary teams.
• Assess member health status, resource needs, services, and treatment plans and provide interventions.
• Facilitate transition into active care management based on member needs.
