Job summary

Centene is seeking a Behavioral Health Care Manager to develop and facilitate complex care management for members with mental health and substance use needs. The role requires a NJ license, a master's in Behavioral Health or Social Work (or a nursing degree), and 2–4 years of related experience.

• Requires Master’s degree in Behavioral Health or Social Work or a nursing degree and 2–4 years of related experience.
• Active New Jersey license is required for this role.
• Licensed Master’s Behavioral Health Professional or RN based on state contract requirements with BH experience

Responsibilities

• Evaluates member needs via phone or in-home visits and develops care plans for behavioral health and social determinants.
• Performs outreach to assess needs and collaborate with resources.
• Coordinates between the member, family/caregivers, resources, and care team to ensure access to services.
• Monitors care plans and outcomes and provides recommendations based on needs.
• Provides education to members and families on procedures and benefits.
• Complies with policies and standards and supports leadership on care quality improvements
