Qualifications

Responsibilities

• Conduct telephonic comprehensive nursing assessments and create care plans.
• Coordinate care with interdisciplinary teams and healthcare providers.
• Monitor patient progress and adjust care plans to improve outcomes.
• Communicate resources available through health plan benefits and community services.
• Document care plans in the electronic health record and participate in monthly meetings.
