Qualifications

Responsibilities

• Meet telephonically with patients and caregivers to discuss care plans.
• Outreach after hospital discharge to assess post-discharge needs.
• Coordinate with physicians and team members to implement care plans.
• Use disease management protocols to optimize outcomes and reduce hospitalizations.
• Monitor and coach patients to maximize self-management.
• Coordinate discharge planning and follow-up services.
• Document care plans and findings in the EHR.
