Job summary

Humana is seeking a Telephonic Care Manager to support a remote case management program. You will coordinate care across sites and levels of care, leveraging resources to prevent readmissions and ensure integrated, compliant care plans for beneficiaries.

Responsibilities include acting as the liaison among CM teams, physicians, and facility staff while pursuing CCM certification within a year and maintaining RN licensure. This role requires 40 hours/week and US citizenship under a DoD contract.

Qualifications

• Active RN license in state of employment.
• Ability to obtain CCM certification within 1 year of hire.
• 2 years case management experience.
• 3 years clinical RN or managed care RN experience.

Responsibilities

• Coordinate and collaborate within Care Management programs to ensure integrated care plans addressing beneficiary needs.
• Serve as the point of contact for complex case management activities across clinical spectrum.
