Job summary

Humana, Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of CenterWell Home Health services.

You will collaborate with the clinical team to ensure appropriate post-acute care and follow-up, with responsibilities spanning care coordination, patient education, and referral management. The role requires an Associate degree (BSN preferred), an active compact RN license, and 3+ years in post-acute nursing.

Qualifications

• Analytical and can problem-solve.
• Excellent verbal and interpersonal skills.
• Must read, write and speak fluent English.
• Current CPR certification.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients who have been discharged from service or for those who may need post-acute services.
• Navigate healthcare options and post-acute offerings, Medicare coverage, billing issues, and accessing healthcare resources.
• Make clinical level of care determination based on discussion, medical records, and other clinical data; match needs to a service site location.
• Maintain awareness of department performance objectives and ensure patient satisfaction goals are met.
• Assist in admissions by acting as an ambassador for patients who meet admissions requirements.
• Place the right patient to the right care setting at the right time.
• Adhere to HIPAA and company training programs and policies.
