Job summary

Molina Healthcare, Inc. seeks an experienced Case Manager for a fully telephonic role. Applicants must reside in Texas and hold LVN, LBSW, or LMSW, or have at least three years of case management experience.

Hours are Monday–Friday, 8 AM–5 PM CST, with 25–40% local travel if required. You will support LTSS care coordination, develop care plans, and collaborate with a multidisciplinary team to ensure quality and cost-effective member care.

Qualifications

• Minimum 2 years health care experience, LTSS exposure preferred.
• Bachelor's degree may be required by some contracts/states.
• Valid driver's license and transportation for travel requirements.

Responsibilities

• Conduct comprehensive member assessments within timelines and may include home visits.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plans and document interventions and outcomes.
• Facilitate interdisciplinary care team meetings for approvals.
• Provide care coordination to address psycho/social, financial, and medical barriers.
